Intermetatarsal ligament breakdown causing splayed toes
Excessive pressure over the metatarsal heads and transverse arch leads to breakdown of intermetatarsal ligaments. In the case pictured above, the ligament between the second and third metatarsals has stretched and the third metatarsal head has subluxed and protrudes through the plantar capsule leading to a "splayed toe sign."
