The electrocardiogram in tetralogy of Fallot shows prominent P waves in V1 (*), suggesting right atrial enlargement. There are also prominent right ventricular forces (tall R waves in V1), an upright T wave in V1, and a marked rightward axis, suggesting right ventricular hypertrophy.
