Contaminated or dirty, infected wound
Groin or below the knee
Wedge excision of lip or ear
Skin flaps on nose
Skin grafting

Prosthetic cardiac valve
Previous IE
Congenital heart disease
Cardiac transplant recipient with valvulopathy
Unrepaired cyanotic congenital heart defects
Repaired congenital heart defects with residual defects at or adjacent to the site of prosthetic patch or device
Congenital heart defect that has been repaired in the previous six months
◊ High risk for hematogenous joint infection:
First two years following joint replacement
Previous prosthetic joint infection
Immunocompromised status
Insulin-dependent diabetes mellitus
HIV infection
Malignancy
Malnutrition
