Double set-up refers to simultaneous preparation for both direct oral intubation and emergency cricothyrotomy.

EGD: extraglottic airway device; IV: intravenous catheter; PTV: percutaneous transtracheal ventilation; RSI: rapid sequence intubation; SaO2: arterial oxygen saturation.

* "Triple setup" refers to simultaneous preparation for direct tracheal intubation, a nonsurgical rescue device (eg, an EGD) if appropriate, and emergency cricothyrotomy.

¶ Signs of significant airway or vascular injury include stridor, drooling, trismus, tracheal deviation, crepitus, expanding hematoma, thrills, bruits, and neurologic deficit consistent with cerebral ischemia. Refer to UpToDate content on airway management in airway trauma and penetrating neck injuries for other signs.
