Tension pneumothorax caused by extensive tuberculosis in the subjacent left lung. Chest radiograph shows a large collection of gas in the left hemithorax with inversion of the left hemidiaphragm and cardiomediastinal shift to the right. The left intercostal spaces are wider than the right ones.
