Lateral radiograph of the cervical spine shows anterior dislocation of C4 on C5 (arrow). Bilateral facet dislocations occur when strong flexion forces extend anteriorly, causing the inferior articulating facets of the upper vertebra to pass over the superior facets of the lower vertebra. Complete spinal cord injury often results.
