This apical four chamber image (inverted) shows the inlet ventricular septal defect (VSD; arrow) at the juncture of the atrioventricular valves. Note that there is no apical displacement of the tricuspid valve hinge point relative to the mitral valve hinge point as is usually seen in normal hearts. With an intact inlet septum, the tricuspid valve is displaced apically when compared with the position of the mitral valve.
