The hernia sac has largely been dissected from the mediastinum allowing the capnoperitoneum to be on the mediastinal side of the sac and decompressing the "spinnaker effect" of the sac. This allows the remaining mediastinal portion of the stomach to easily and safely reduce back into the abdomen.
