Magnetic resonance imaging findings in a patient with chronic cryptococcal meningoencephalitis
64-year-old male presenting with 7 months of bilateral lower extremity sensory changes without weakness. T1-weighted postcontrast sagittal (A) and axial (B) spine MRIs show diffuse lumbosacral leptomeningeal enhancement involving the entire thecal sac. T1-weighted postcontrast sagittal (C) and axial (D) brain MRI show an enhancing intracranial nodule in the posterior fossa (arrows). CSF showed low glucose (37 mg/dL), high protein (170 mg/dL), elevated RBCs (1060 cells/microL), increased nucleated cells (79 cells/microL with a neutrophilic predominance), positive oligoclonal bands, elevated ACE level (4.3), and positive cryptococcal antigen (CrAg).
