Newly diagnosed resectable endometrial cancer: Approach to risk stratification after surgery

Newly diagnosed resectable endometrial cancer: Approach to risk stratification after surgery

Endometrial carcinoma is surgically staged; the staging system is presented in detail separately.

Patients with local/regional spread, spread to the bladder or intestinal mucosa, or distant metastases are discussed elsewhere.

LVSI: lymphovascular space invasion.

* Nonendometrioid tumors include serous, clear cell, carcinosarcoma, mixed, undifferentiated, and mesonephric-like carcinomas.

¶ Definitions of grade:
  • Grade 1: Less than 5% solid growth patterns
  • Grade 2: 6 to 50% solid growth patterns
  • Grade 3: Greater than 50% solid growth

Δ Molecular factors (eg, DNA polymerase epsilon mutated [POLEmut], mismatch repair deficient [MMRd], no specific molecular profile [NSMP], p53 abnormal [p53abn]), if obtained, can be used to up-stage or down-stage early endometrial carcinomas and may help inform prognosis and guide treatment decisions.
