COPD: chronic obstructive pulmonary disease; CT: computed tomography.

(A) 62-year-old male with a history of COPD and bronchiectasis presents with malaise and productive cough. CT scan shows a left upper lobe cavity with intracavitary material. The diagnosis of chronic cavitary pulmonary aspergillosis was made. Voriconazole was started.

(B) CT scan 6 months after starting voriconazole. The cavity appears more thin-walled, suggesting good response to therapy but continued evidence of ongoing disease.
