Axial (A) and coronal (B) high-resolution computed tomography (CT) images demonstrate conglomerate bilateral upper lobe fibrosis, marked upper lobe volume loss, and multiple subpleural bullae. The patient was a 56-year-old life-long nonsmoker with longstanding severe sarcoidosis.
