Images are from a 29-year-old male with HIV infection and pulmonary TB who presented with left-sided abdominal pain.
  • Panel A – CT of the abdomen showing numerous hypodense lesions
  • Panel B – Splenectomy specimen showing necrotic nodules with purulent discharge
  • Panel C – Histology showing granulomatous inflammation with caseous necrosis
  • Panel D – AFB staining (arrowheads show bacilli)

Real-time PCR identified Mycobacterium tuberculosis. The patient recovered with anti-TB therapy.
