Hospitals often require nurse administered pain medication orders to include a pain level threshold and recommend IV opioid use for severe pain (eg, 8 to 10 on a 0 to 10 pain assessment scale).

¶ After initial two hourly doses (1.5 mg and 0.75 mg if needed), oliceridine dose may be repeated at intervals of no less than one hour as needed and titrated based on response and tolerability to a maximum of 3 mg per dose; maximum cumulative dose is 27 mg in 24 hours.
