* Erythrocytosis: Hgb >16.5 g/dL/Hct >49% in euvolemic men or Hgb >16 g/dL/Hct >48% in euvolemic women.

¶ Test for exon 14 and exon 12 mutations; variant allele frequency >1% is considered positive.
