82-year-old with lung cancer metastatic to brain. Postcontrast T1-weighted MRI (A) shows a 2 cm, solidly enhancing lesion in the right insula. FLAIR image (B) shows extensive associated hyperintense vasogenic edema and mass effect that far exceeds the size of the metastasis itself (arrow).
