This video shows ultrasound guided transgluteal sciatic nerve block, as follows:
- Place the transducer midway between the greater trochanter and the posterior superior iliac spine with the ultrasound beam perpendicular to the femur.
- Insert the needle in plane, in a lateral to medial orientation, through the gluteus maximus muscle. Out of plane needle insertion may also be used.
- Place the needle tip adjacent to the lateral aspect of the sciatic nerve. Inject a small amount of saline to visualize spread above or below the nerve.
- After negative aspiration, inject approximately 20 mL of local anesthetic in 5 mL increments, with gentle aspiration between injections. Visualize spread of local anesthetic around the nerve.
