Missed dose: If a sotorasib dose is missed by >6 hours, administer the next dose as scheduled the following day; do not administer 2 doses at the same time. If vomiting occurs, do not administer an additional dose. Resume dosing as scheduled the following day.
eGFR ≥30 mL/minute/1.73 m2: There are no dosage adjustments provided in the manufacturer’s labeling; however, no clinically meaningful differences in sotorasib pharmacokinetics were observed in mild and moderate kidney impairment.

Hepatic impairment prior to treatment initiation:
Mild to moderate impairment (Child-Turcotte-Pugh class A, B): No dosage adjustment necessary.

Hepatotoxicity during treatment:
Grade 2 AST or ALT elevation (with symptoms) or grade 3 or 4 AST or ALT elevation: Withhold sotorasib until recovery to ≤ grade 1 or baseline, then resume at the next lower dose level (see "Dosing: Adjustment for Toxicity" for dose reduction recommendations).
AST or ALT >3 times ULN with total bilirubin >2 times ULN (in the absence of alternative etiologies): Permanently discontinue sotorasib.

Concerns related to adverse effects:
• Hepatotoxicity: Sotorasib may cause hepatotoxicity, which may result in drug-induced liver injury and hepatitis. In a clinical trial, increased ALT and/or AST were observed, including grade 3 and 4 events. The median time to first onset of elevated ALT and/or AST was 9 weeks (range: 0.3 to 42 weeks). Some patients received corticosteroids for management of hepatotoxicity.
• Pulmonary toxicity: Interstitial lung disease (ILD)/pneumonitis may occur, and could be fatal. In a clinical trial, ILD/pneumonitis occurred rarely; all cases were grade 3 or 4 at onset, and a fatality did occur. The median time to first onset of ILD/pneumonitis was 2 weeks (range: 2 to 18 weeks).

Oral: Administer at the same time each day with or without food. Swallow whole; do not chew, crush, or split tablets.
If unable to swallow tablets, an oral mixture may be prepared. Disperse tablets in 120 mL of noncarbonated, room temperature water (do not crush tablets); do not use other liquids. Stir or swirl the cup for ~3 minutes until tablets are dispersed into small pieces; the tablets will not completely dissolve (the mixture may appear pale yellow to bright yellow). Drink immediately or within 2 hours of dispersion; do not chew tablet pieces. Rinse the container with an additional 120 mL of noncarbonated water and swallow. If the mixture is not administered immediately, stir again to ensure that the tablets are dispersed.

In animal reproduction studies, maternal toxicity was observed in rats and rabbits administered sotorasib in doses equivalent to ~4.6 times and ~2.6 times the human dose, respectively (based on AUC). Embryo-fetal adverse events were not observed in the rat studies at this dose level; decreased fetal body weight and a reduced number of ossified metacarpals were observed in the fetuses of rabbits.
It is not known if sotorasib is present in breast milk.
Due to the potential for serious adverse reactions in the breastfed infant, breastfeeding is not recommended by the manufacturer during therapy and for 1 week after the last dose of sotorasib.
KRAS G12C mutation status (in tumor or plasma specimens; if no mutation is detected in a plasma specimen, test tumor tissue). Monitor LFTs (ALT, AST, and total bilirubin) prior to sotorasib initiation, every 3 weeks for the first 3 months, then once a month or as clinically indicated (test more frequently in patients who develop transaminase and/or bilirubin elevations). Monitor for signs/symptoms of interstitial lung disease/pneumonitis (eg, dyspnea, cough, fever). Monitor adherence.
The American Society of Clinical Oncology hepatitis B virus (HBV) screening and management provisional clinical opinion (ASCO [Hwang 2020]) recommends HBV screening with hepatitis B surface antigen, hepatitis B core antibody, total Ig or IgG, and antibody to hepatitis B surface antigen prior to beginning (or at the beginning of) systemic anticancer therapy; do not delay treatment for screening/results. Detection of chronic or past HBV infection requires a risk assessment to determine antiviral prophylaxis requirements, monitoring, and follow-up.
Sotorasib inhibits KRAS G12C, which is a tumor-restricted, mutant-oncogenic form of KRAS. Sotorasib forms a covalent irreversible bond with the unique cysteine of KRAS G12C, locking the protein in an inactive state that prevents downstream signaling; wild-type KRAS is not affected. Sotorasib displays minimal off-target activity; it blocked KRAS signaling, inhibited cell growth, and promoted apoptosis in KRAS G12C cell lines only. In KRAS G12C animal models, sotorasib treatment resulted in tumor regression and prolonged survival (and was associated with antitumor immunity).
Absorption: Sotorasib AUC0-24h increased by 25% when administered with a high-fat, high-calorie meal (~800 to 1,000 calories with 150, 250, and 500 to 600 calories from protein, carbohydrate, and fat, respectively) compared to the fasted state.
