Pexidartinib can cause serious and potentially fatal liver injury. Monitor liver tests prior to initiation of pexidartinib and at specified intervals during treatment. Withhold and dose reduce or permanently discontinue pexidartinib based on severity of hepatotoxicity.

Tenosynovial giant cell tumor, symptomatic: Oral: 250 mg twice daily with a low-fat meal (~11 to 14 g total fat) until disease progression or unacceptable toxicity. Administration with a high-fat meal (~55 to 65 g total fat) increases pexidartinib concentrations and may increase the risk for adverse reactions, including hepatotoxicity.
Missed dose: If a dose is missed or vomited, administer the next dose at its scheduled time.

Concerns related to adverse effects:
• Hepatotoxicity: Pexidartinib can cause serious and potentially fatal liver injury. Hepatotoxicity with ductopenia and cholestasis has occurred, including rare irreversible cases of cholestatic liver injury (which were fatal or required liver transplantation). The occurrence of pexidartinib-related cholestatic hepatoxicity cannot be predicted, and the mechanism of hepatotoxicity is unknown. It is not known if liver injury occurs in the absence of transaminase elevations. A small percentage of patients in a clinical study who received pexidartinib developed signs of serious liver injury (ALT or AST ≥3 × ULN with total bilirubin ≥2 × ULN). In these patients, peak ALT ranged from 6 to 9 × ULN, peak total bilirubin ranged from 2.5 to 15 × ULN, and alkaline phosphatase was ≥2 × ULN. ALT, AST, and total bilirubin improved to <2 × ULN at 1 to 7 months after discontinuing pexidartinib. Avoid pexidartinib in patients with preexisting increased serum transaminases, total bilirubin or direct bilirubin (> ULN), or active liver or biliary tract disease (including increased alkaline phosphatase). Avoid concurrent administration of other hepatotoxic medications in patients with elevated transaminases, elevated total or direct bilirubin (> ULN), or active liver or biliary tract disease.
• Photosensitivity: Patients should avoid prolonged sun exposure during treatment; advise patients to wear protective clothing and use broad-spectrum UVA/UVB sunscreen.

Administration of pexidartinib 400 mg with a high-fat meal (800 to 1,000 calories with fat as 50% of caloric content [~55 to 65 g of total fat]) increased the Cmax and AUC by 100% and delayed the time to peak by 2.5 hours; administration of pexidartinib 400 mg with a low-fat meal (387 calories with fat ~25% of caloric intake [~11 to 14 g of total fat]) increased the Cmax and AUC by 56% and 59%, respectively, and delayed the time to peak by 1.5 hours. Grapefruit juice may increase pexidartinib serum concentrations. Management: Administer capsules with a low-fat meal (~11 to 14 g of total fat). Avoid grapefruit juice.
Evaluate pregnancy status prior to treatment initiation in patients who could become pregnant. Patients who could become pregnant should use effective nonhormonal contraception during treatment and for 1 month after the last pexidartinib dose. Patients with partners who could become pregnant should use effective contraception during treatment and for 1 week after the last pexidartinib dose.
Based on the mechanism of action and data from animal reproduction studies, in utero exposure to pexidartinib may cause fetal harm.
It is not known if pexidartinib is present in breast milk.
Due to the potential for serious adverse reactions in the breastfed infant, breastfeeding is not recommended by the manufacturer during treatment and for at least 1 week after the last pexidartinib dose.
Avoid grapefruit juice.
Monitor LFTs (including AST, ALT, total bilirubin, direct bilirubin, alkaline phosphatase, and gamma-glutamyl transferase [GGT]) prior to pexidartinib initiation, weekly for the first 8 weeks, every 2 weeks for the next month, and then every 3 months thereafter; monitor more frequently in patients with hepatotoxicity; following rechallenge, monitor LFTs weekly for the first month. Monitor kidney function. Evaluate pregnancy status prior to treatment initiation (in patients who could become pregnant). Monitor for signs/symptoms of liver injury. Monitor adherence.
The American Society of Clinical Oncology hepatitis B virus (HBV) screening and management provisional clinical opinion (ASCO [Hwang 2020]) recommends HBV screening with hepatitis B surface antigen, hepatitis B core antibody, total Ig or IgG, and antibody to hepatitis B surface antigen prior to beginning (or at the beginning of) systemic anticancer therapy; do not delay treatment for screening/results. Detection of chronic or past HBV infection requires a risk assessment to determine antiviral prophylaxis requirements, monitoring, and follow-up.
Pexidartinib is a tyrosine kinase inhibitor with selective and strong inhibitory activity against the colony-stimulating factor 1 receptor (CSF1R); pexidartinib also inhibits KIT and FMS-like tyrosine kinase 3 (FLT3)-internal tandem duplication (Tap 2019). Overexpression of the CSF1R ligand promotes cell proliferation and accumulation in the synovium. Pexidartinib inhibits proliferation of a CSF1R dependent cell line.
Pharmacokinetic note: According to the manufacturer, there is no clinically significant difference in relative exposure between administration of 250 mg twice daily with a low-fat meal and 400 mg twice daily on an empty stomach.

Protein binding: >99%; to albumin and alpha-1 acid glycoprotein.
Metabolism: Oxidation via CYP3A4 and glucuronidation via UGT1A4 (glucuronidation forms inactive N-glucuronide metabolite).

Altered kidney function: Mild, moderate, and severe impairment (CrCl 15 to 89 mL/minute) increased pexidartinib exposure by ~30%, compared to normal kidney function (CrCl ≥90 mL/minute).
Hepatic function impairment: Moderate impairment (1.5 to 3 times ULN [not due to Gilbert syndrome] and any AST) increased pexidartinib exposure by 43% compared to normal hepatic function (total bilirubin and AST ≤ ULN).
