Knowledge of the relationship of the heart to other structures is important for assessing traumatic cardiac injury. Anterior to the majority of the heart is the sternum, and posteriorly the spine, esophagus, and descending thoracic aorta are in close association. The diaphragmatic surface of the heart is occupied by one third of the right ventricle and two thirds of the left ventricle.
