* Imaging with whole body FDG PET/CT is preferred at diagnosis and for response assessment. CT of the chest, abdomen, and pelvis with contrast is used for annual surveillance.
¶ Serum biomarkers include complete blood count, blood chemistries, and C-reactive protein.
