Testicular torsion
(A) Note that the left testicle is high riding. This patient had a 12 hour history of pain with vomiting and had no cremasteric reflex on examination.
(B) At surgery, the patient had torsion with ischemia of the left testicle. Contralateral exploration demonstrated a bell clapper deformity on the right side.
