Screening for colorectal cancer (CRC) is effective in reducing CRC related mortality. Current screening methods include endoscopy based and biomarker based approaches. This guideline is a joint official statement of the Asian Pacific Association of Gastroenterology (APAGE) and the Asian Pacific Society of Digestive Endoscopy (APSDE), developed in response to the increasing use of, and accumulating supportive evidence for the role of, non-invasive biomarkers for the diagnosis of CRC and its precursor lesions.

Helicobacter pyloriتpromotes colorectal carcinogenesis by deregulating intestinal immunity and inducing a mucus-degrading microbiota signature

AnnaتRalser,تAlisaتDietl,تSebastianتJarosch,تVeronikaتEngelsberger,تAndreasتWanisch,تKlaus PeterتJanssen,تMoritzتMiddelhoff,تMichaelتVieth,تMichaelتQuante,تDirkتHaller,تDirk HتBusch,تLiتDeng,تRaquelتMej’as-Luque,تMarkusتGerhard

doi : 10.1136/gutjnl-2022-328075

ObjectiveتHelicobacter pyloriتinfection is the most prevalent bacterial infection worldwide. Besides being the most important risk factor for gastric cancer development, epidemiological data show that infected individuals harbour a nearly twofold increased risk to develop colorectal cancer (CRC). However, a direct causal and functional connection betweenتH. pyloriتinfection and colon cancer is lacking.

doi : 10.1136/gutjnl-2022-327337

ObjectiveتThe extent to which tryptophan (Trp) metabolism alterations explain or influence the outcome of inflammatory bowel diseases (IBDs) is still unclear. However, several Trp metabolism end-products are essential to intestinal homeostasis. Here, we investigated the role of metabolites from the kynurenine pathway.

Acetyltransferase fromتAkkermansia muciniphilaتblunts colorectal tumourigenesis by reprogramming tumour microenvironment

YiتJiang,تYuejieتXu,تChangتZheng,تLeiتYe,تPingتJiang,تSaraتMalik,تGuifangتXu,تQianتZhou,تMingmingتZhang

doi : 10.1136/gutjnl-2022-327853

ObjectiveتThe protein post-translational modification (PTM) in host cells can be rewritten by bacterial enzymes and represents an unprecedented mechanism in the communication between intestinal flora and the host. AlthoughتAkkermansia muciniphilaتhas been widely investigated as a probiotic and blunts colitis-associated tumourigenesis in mice, there is little understanding regarding whetherتA. muciniphilaتis involved in the PTM of colorectal cancer (CRC). This study investigates whether and howتA. muciniphilaتengages in the PTM of host CRC.

doi : 10.1136/gutjnl-2022-327608

ObjectiveتBiological insights into the stepwise development and progression of colorectal cancer (CRC) are imperative to develop tailored approaches for early detection and optimal clinical management of this disease. Here, we aimed to dissect the transcriptional and immunologic alterations that accompany malignant transformation in CRC and to identify clinically relevant biomarkers through spatial profiling of pT1 CRC samples.

doi : 10.1136/gutjnl-2022-327960

ObjectiveتIncreasing evidence implicates mutation-induced protein misfolding and endoplasm reticulum (ER) stress in the pathophysiology of chronic pancreatitis (CP). The paucity of animal models harbouring genetic risk variants has hampered our understanding of how misfolded proteins trigger CP.

Activated regulatory T-cells promote duodenal bacterial translocation into necrotic areas in severe acute pancreatitis

doi : 10.1136/gutjnl-2022-327448

ObjectiveتIn acute pancreatitis (AP), bacterial translocation and subsequent infection of pancreatic necrosis are the main risk factors for severe disease and late death. Understanding how immunological host defence mechanisms fail to protect the intestinal barrier is of great importance in reducing the mortality risk of the disease.ت

S100A10 promotes HCC development and progression via transfer in extracellular vesicles and regulating their protein cargos

XiaتWang,تHongyangتHuang,تKaren Man-FongتSze,تJinتWang,تLuتTian,تJingyiتLu,تYu-ManتTsui,تHoi TangتMa,تEvaتLee,تAoتChen,تJoyceتLee,تYingتWang,تJudy Wai PingتYam,تTan-ToتCheung,تXinyuanتGuan,تIrene Oi-LinتNg

doi : 10.1136/gutjnl-2022-327998

ObjectiveتGrowing evidence indicates that tumour cells exhibit characteristics similar to their lineage progenitor cells. We found that S100 calcium binding protein A10 (S100A10) exhibited an expression pattern similar to that of liver progenitor genes. However, the role of S100A10 in hepatocellular carcinoma (HCC) progression is unclear. Furthermore, extracellular vesicles (EVs) are critical mediators of tumourigenesis and metastasis, but the extracellular functions of S100A10, particularly those related to EVs (EV-S100A10), are unknown.

doi : 10.1136/gutjnl-2022-328689

ObjectiveتA simple combined score with liver stiffness, controlled attenuation parameter and serum aspartate aminotransferase (AST), the FibroScan-AST (FAST) score, has been proposed to non-invasively identify patients with fibrotic non-alcoholic steatohepatitis (NASH). We performed a systematic review and meta-analysis of published studies to evaluate the overall diagnostic accuracy of the FAST score in identifying patients with fibrotic NASH.
