Thomas A. Dewland, MD, Edward P. Gerstenfeld, MD, Joshua D. Moss, MD, Adam C. Lee, MBBS, Vasanth Vedantham, MD, PHD, Randall J. Lee, MD, PHD, Zian H. Tseng, MD, MAS, Henry H. Hsia, MD, Byron K. Lee, MD, MAS, Grace C. Wall, BA, Kathleen R. Chang, BS, Michelle H. Yang, BS, Gregory M. Marcus, MD, MAS

doi : 10.1016/j.jacep.2022.10.017

Volume 9, Issue 5, May 2023, Pages 611-619

Transseptal puncture is a necessary component of many electrophysiology and structural heart procedures. Improving this technique has broad ramifications for the overall efficiency and safety of these interventions. A new technology uses a specialized introducer wire to cross the septum with radiofrequency (RF) energy, eliminating the need for a transseptal needle and wire/needle exchanges.

Left Bundle Branch Area Defibrillator (LBBAD): A First-in-Human Feasibility Study

Wim L.H. Huybrechts, MD, a,b Marco Bergonti, MD,a Johan B. Saenen, MD, PHD, a,b Hielko Miljoen, MD, a,b Olivier Van Leuven, MD, a Lauranne Van Assche, MD, a Paul Van Herck, MD, P HD, a Anouk Wittock, MD,c Dieter De Cleen, MD, d Andrea Sarkozy, MD, PHD, a,b Hein Heidbüchel, MD, PH D

doi : 10.1016/j.jacep.2023.01.006

Volume 9, Issue 5, May 2023, Pages 620-627

Left bundle branch area (LBBA) pacing is a promising pacing technique. LBBA implantable cardioverter-defibrillator (ICD) lead implantation reduces the number of leads in patients with both pacing and ICD indications, reducing cost and potentially increasing safety. LBBA positioning of ICD leads has not previously been described.

doi : 10.1016/j.jacep.2023.01.014

Volume 9, Issue 5, May 2023, Pages 628-637

Prior studies have demonstrated clinical benefits associated with cryoballoon pulmonary vein isolation (PVI) and concomitant posterior wall isolation (PWI) in patients with persistent atrial fibrillation (AF). However, the role for this approach in patients with paroxysmal atrial fibrillation (PAF) remains unclear.

Failure of Intrinsic Antitachycardia Pacing to Terminate Ventricular Tachycardia and Potential Proarrhythmic Effects

Anvi Raina, MD, Martin van Zyl, MB BC H, Mark Henrich, RN, Abhishek J. Deshmukh, MBBS, Gurukripa N. Kowlgi, MBBS

Volume 9, Issue 5, May 2023, Pages 697-700

Intrinsic antitachycardia pacing (iATP) is a novel, automated antitachycardia pacing (ATP) algorithm that provides individualized therapy to terminate ventricular tachycardia (VT). If the first ATP attempt is unsuccessful, the algorithm analyzes the tachycardia cycle length and the postpacing interval and adjusts the subsequent sequence to successfully terminate VT.
