Pre-operative N-terminal pro-B-type natriuretic peptide for prediction of acute kidney injury after noncardiac surgery: A retrospective cohort study

Zhao, Bing-Cheng; Zhuang, Pei-Pei; Lei, Shao-Hui; Qiu, Shi-Da; Yang, Xiao; Li, Cai; Liu, Wei-Feng; Liu, Ke-Xuan

European Journal of Anaesthesiology. 38(6):591-599, June 2021.

Acute kidney injury (AKI) is associated with poor outcomes after noncardiac surgery. Whether pre-operative N-terminal pro-B-type natriuretic peptide (NT-proBNP) predicts AKI after noncardiac surgery is unclear.

Expert consensus on peri-operative myocardial injury screening in noncardiac surgery: A literature review

Puelacher, Christian; Bollen Pinto, Bernardo; Mills, Nicholas L.; Duceppe, Emmanuelle; Popova, Ekaterine; Duma, Andreas; Nagele, Peter; Omland, Torbj?rn; Hammerer-Lercher, Angelika; Lurati Buse, Giovanna

European Journal of Anaesthesiology. 38(6):600-608, June 2021.

Peri-operative myocardial injury, detected by dynamic and elevated cardiac troponin (cTn) concentrations, is a common complication of noncardiac surgery that is strongly associated with 30-day mortality. Although active screening for peri-operative myocardial injury has been suggested in recent guidelines, clinical implementation remains tentative due to a lack of examples on how to tackle such an interdisciplinary project at a local level. Moreover, consensus on which assay and cTn cut-off values should be used has not yet been reached, and guidance on whom to screen is lacking. In this article, we aim to summarise local examples of successfully implemented cTn screening practices and review the current literature in order to provide information and suggestions for patient selection, organisation of a screening programme, caveats and a potential management pathway.

Clinical performance of a machine-learning algorithm to predict intra-operative hypotension with noninvasive arterial pressure waveforms: A cohort study

Wijnberge, Marije; van der Ster, Bj?rn J.P.; Geerts, Bart F.; de Beer, Friso; Beurskens, Charlotte; Emal, Dina; Hollmann, Markus W.; Vlaar, Alexander P.J.; Veelo, Denise P

doi : 10.1097/EJA.0000000000001521

European Journal of Anaesthesiology. 38(6):609-615, June 2021.

Intra-operative hypotension is associated with adverse postoperative outcomes. A machine-learning-derived algorithm developed to predict hypotension based on arterial blood pressure (ABP) waveforms significantly reduced intra-operative hypotension. The algorithm calculates the likelihood of hypotension occurring within minutes, expressed as the Hypotension Prediction Index (HPI) which ranges from 0 to 100. Currently, HPI is only available for patients monitored with invasive ABP, which is restricted to high-risk procedures and patients. In this study, the performance of HPI, employing noninvasive continuous ABP measurements, is assessed.

Hidden sources of fluids, sodium and potassium in stabilised Swedish ICU patients: A multicentre retrospective observational study

Nihlén, Sandra; Kawati, Rafael; Rasmusson, Johan; Rylander, Christian; Pikwer, Andreas; Castegren, Markus; Belin, Anton; Lipcsey, Miklos

doi : 10.1097/EJA.0000000000001354

European Journal of Anaesthesiology. 38(6):625-633, June 2021.

Fluid overload in ICU patients is associated with increased morbidity and mortality. Although studies report on optimisation of resuscitation fluids given to ICU patients, increasing evidence suggests that maintenance fluids and fluids used to administer drugs are important sources of fluid overload.

Effects of two stepwise lung recruitment strategies on respiratory function and haemodynamics in anaesthetised pigs: A randomised crossover study

Wittenstein, Jakob; Huhle, Robert; Scharffenberg, Martin; Kiss, Thomas; Herold, Johannes; Vivona, Luigi; Bergamaschi, Alice; Schultz, Marcus J.; Pelosi, Paolo; Gama de Abreu, Marcelo; Bluth, Thomas

doi : 10.1097/EJA.0000000000001480

European Journal of Anaesthesiology. 38(6):634-643, June 2021.

Lung recruitment manoeuvres and positive end-expiratory pressure (PEEP) can improve lung function during general anaesthesia. Different recruitment manoeuvre strategies have been described in large international trials: in the protective ventilation using high vs. low PEEP (PROVHILO) strategy, tidal volume ( V T ) was increased during volume-controlled ventilation; in the individualised peri-operative open-lung approach vs. standard protective ventilation in abdominal surgery (iPROVE) strategy, PEEP was increased during pressure-controlled ventilation.
