Aerobic exercise has become a useful method to assist with postconcussion management. Exercise can exacerbate concussion symptoms even when symptoms are not apparent at rest. Few studies have examined the reasons for symptom exacerbation during exercise following a concussion. We had 2 primary objectives: (1) to delineate cardiopulmonary and cerebrovascular responses to exercise in adolescents and young adults with a concussion and healthy controls and (2) to determine the association between cerebrovascular responses and symptom burden.

Fish intake may prevent cerebrovascular disease (CVD), yet the mechanisms are unclear, especially regarding its impact on subclinical damage. Assuming that fish may have pleiotropic effect on cerebrovascular health, we investigated the association of fish intake with global CVD burden based on brain MRI markers.

In 1885, while working at the University of Pennsylvania, Eadweard Muybridge and Francis Dercum used nascent photographic technology to perform the first-ever motion picture study of neurologic patients. To date, our clinical understanding of the Muybridge-Dercum project has been limited to those clinical details included in a handful of contemporaneous publications by Dercum et al. In this study, recently rediscovered clinical notebooks from the Hospital of the University of Pennsylvania's Dispensary for Nervous Diseases were reviewed and found to contain the original clinical records of 9 of Muybridge and Dercum's photographic subjects. These records add new clinical insights into our understanding of this historic photographic study and revive the zeitgeist of a foundational period in the development of neurology as a medical subspecialty in the United States.

Hereditary hyperekplexia is a rare neurologic disorder characterized by an exaggerated startle response with profound muscle stiffness.1,2 Given the nature of the spells, this condition is often misdiagnosed as epilepsy. Mutations in glycine receptors and transporters are the primary cause of this syndrome.1 We present an example of stimulus-induced hyperekplexia captured on video EEG in a 7-week-old girl with compound heterozygous variants in the presynaptic glycine transporter gene SLC6A5.
