In refractory cases, hydroxychloroquine (HCQ) can be added to this standard of care.

Comparison of the ‘4-item assessment test’ and ‘nursing delirium screening scale’ delirium screening tools on non-intensive care unit wards: A prospective mixed-method approach

Zastrow, Inke; Tohsche, Peter; Loewen, Theresa; Vogt, Birgit; Feige, Melanie; Behnke, Martina; Wolff, Antje; Kiefmann, Rainer; Olotu, Cynthia

doi : 10.1097/EJA.0000000000001470

September 2021 - Volume 38 - Issue 9 - p 957-965

In elderly patients following surgery, postoperative delirium (POD) is the most frequent complication and is associated with negative outcomes. The 2017 European Society of Anaesthesiology guideline on POD aims to improve patient care by implementing structured delirium prevention, diagnosis and treatment. However, these recommendations, especially systematic delirium screening, are still incompletely adopted in clinical practice. The aim of this study was to evaluate the feasibility and acceptance of validated delirium screening tools and to identify barriers to their implementation on nonintensive care unit wards.

September 2021 - Volume 38 - Issue 9 - p 966-974

Near-infrared spectroscopy (NIRS) is used routinely to monitor cerebral tissue oxygen saturation (SctO2) during cardiopulmonary bypass (CPB) but is rarely employed outside the operating room. Previous studies indicate that patients are at risk of postoperative cerebral oxygen desaturation after cardiac surgery.

Effects of pupillary reflex dilation-guided opioid administration on remifentanil and morphine consumption during laparoscopic surgery: A randomised controlled trial

Guerci, Philippe; Jay, Guillaume; Arnout, Chloé; Herbain, Delphine; Baka, Noureddine; Poirel, Olivier; Novy, Emmanuel; Bouaziz, Hervé; Vial, Florence

doi : 10.1097/EJA.0000000000001491

September 2021 - Volume 38 - Issue 9 - p 975-984

Analysis of pupillary reflex dilation (PRD) assesses the balance of nociception--antinociception. Laparoscopic surgery induces haemodynamic variations that are misleading. During laparoscopy, PRD guidance helps differentiate haemodynamic changes because of excess nociception from secondary changes related to the reflex release of endocrine factors.

Pain management after complex spine surgery: A systematic review and procedure-specific postoperative pain management recommendations

Waelkens, Piet; Alsabbagh, Emissia; Sauter, Axel; Joshi, Girish P.; Beloeil, Hélène; on behalf of the PROSPECT Working group?? of the European Society of Regional Anaesthesia and Pain therapy (ESRA)

doi : 10.1097/EJA.0000000000001448

September 2021 - Volume 38 - Issue 9 - p 985-994

Complex spinal procedures are associated with intense pain in the postoperative period. Adequate peri-operative pain management has been shown to correlate with improved outcomes including early ambulation and early discharge.

Tapentadol versus oxycodone analgesia and side effects after laparoscopic hysterectomy: A randomised controlled trial

Comelon, Marlin; Raeder, Johan; Dr?gni, Tomas; Lieng, Marit; Lenz, Harald

doi : 10.1097/EJA.0000000000001425

September 2021 - Volume 38 - Issue 9 - p 995-1002

Tapentadol is an opioid, which acts as a ?-opioid receptor agonist and inhibits noradrenaline reuptake in the central nervous system. This dual mechanism of action results in synergistic analgesic effects and potentially less side effects. This has been shown in treatment of chronic pain but postoperative studies are sparse.
