Economic analysis of prenatal fetoscopic vs open-hysterotomy repair of open neural tube defect

B. C. King,J. Hagan,R. Corroenne,A. A. Shamshirsaz,J. Espinoza,A. A. Nassr,W. Whitehead,M. A. Belfort,M. Sanz Cortes

doi : 10.1002/uog.22089

Volume 58, Issue 2 p. 230-237

Fetal repair of an open neural tube defect (ONTD) by open hysterotomy has been shown to reduce the need for ventriculoperitoneal shunting and improve motor outcomes for infants, but increases the risk of Cesarean section and prematurity. Fetoscopic repair is an alternative approach that may confer similar neurological benefits but allows for vaginal delivery and reduces the incidence of hysterotomy-related complications. We sought to compare the costs of care from fetal surgery until neonatal discharge, as well as the clinical outcomes, associated with each surgical approach.

Fetal–placental blood flow and neurodevelopment in childhood: population-based neuroimaging study

S. Sammallahti,H. Tiemeier,S. Louwen,E. Steegers,M. Hillegers,V. W. V. Jaddoe,T. White

doi : 10.1002/uog.22185

Volume 58, Issue 2 p. 245-253

Antenatal Doppler measurements of the fetal umbilical and cerebral circulations can predict perinatal complications; however, it is unclear if subtle variations in antenatal Doppler measurements are associated with long-term neurodevelopmental outcome. In this study, we examined whether antenatal Doppler measurements of the fetal–placental circulation are associated with cognitive and motor abilities and brain morphology in childhood.

Three-dimensional echocardiography and cardiac strain imaging in women with gestational diabetes mellitus

A. M. Company Calabuig,E. Nunez,A. S?nchez,K. H. Nicolaides,M. Charakida,C. De Paco Matallana

doi : 10.1002/uog.23666

Volume 58, Issue 2 p. 278-284

Gestational diabetes mellitus (GDM) is associated with premature cardiovascular disease and adverse cardiovascular outcome in the mother. Subclinical cardiac functional changes in the left ventricle have been reported during pregnancy in women with GDM using conventional echocardiography, but results are inconsistent. The aims of the current study were to assess whether GDM is associated with biventricular systolic dysfunction in the mother and whether these cardiac changes can be detected using the novel echocardiographic modalities of strain imaging and three-dimensional (3D) echocardiography.

Value of cervical electrical impedance spectroscopy to predict spontaneous preterm delivery in asymptomatic women: the ECCLIPPx prospective cohort study

D. O. C. Anumba,V. Stern,J. T. Healey,S. Dixon,B. H. Brown

doi : 10.1002/uog.22180

Volume 58, Issue 2 p. 293-302

Preterm birth (PTB) accounts for two-thirds of deaths of structurally normal babies and is associated with substantial lifetime healthcare costs. Prevention of PTB remains limited by the modest accuracy of prediction methods, namely transvaginal ultrasound (TVS) cervical length (CL) measurement and quantitative cervicovaginal fetal fibronectin (FFN) estimation. We report the first substantive study detailing the predictive performance of a cervical probe device based on electrical impedance spectroscopy (EIS) for PTB – the EleCtriCaL Impedance Prediction of Preterm birth by spectroscopy of the cervix (ECCLIPPx) study. We aimed to compare the accuracy of cervical EIS-based prediction of spontaneous PTB with that of prediction using TVS-CL and FFN in asymptomatic women in the mid-trimester.
